St. Philip's Prayer Discipline Application

To apply for membership in the Prayer Discipline, print and fill out the application form. Mail the form to the address below along with check of $25.00 payable to The Fellowship of St. John the Divine, to help defray the expenses of the Prayer Discipline. In lieu of mailing a check, you can donate $25.00 online using a credit card.

Continuing members are encouraged to send $10 each year to further the ministry!
